How to Read a Medicare Summary Notice and Spot a Billing Problem
People with Original Medicare receive Medicare Summary Notices (MSNs) showing services or supplies billed to Medicare. The notice is an important tool for catching errors and understanding what Medicare paid.
An MSN is not itself a bill
It summarizes claims and shows what the provider charged, what Medicare approved and what you may owe. Compare it with actual provider bills before paying.
Review services you recognize
Check dates, provider names and descriptions. If a service appears that you did not receive, contact the provider and Medicare using official contact information.
Look at the notes
Claim decisions include codes and explanations. If Medicare did not pay, the notice can explain why and provide information about appeal rights.
QMB beneficiaries have special protections
If you are in the Qualified Medicare Beneficiary program, providers generally cannot bill you for Medicare-covered deductibles, coinsurance and copayments. Keep documentation of QMB status and ask for help if you are improperly billed.
